How do I get a prescription refill? |
| A. |
At the time that you see the physician either in the hospital, at the time of discharge or in clinical evaluation in Shore Heart Group offices, typically prescriptions are written for quantity #90 and 3 refills. If you are going to run out of your prescription, please call your pharmacist for all prescription refills. Shore Heart Group will authorize refills only during routine office hours and only if you are actively under our care. Please try to anticipate your needs for weekends, holidays or vacations. |

How do I communicate with Shore Heart Group office in order to schedule an appointment or diagnostic test? |

At the time of scheduling a diagnostic procedure, all patients are given a written description of the test and instructions pertinent to that procedure. Diagnostic testing results, consultation reports and reports on follow-up visits will be mailed to the patient’s referring physician 48 hours after the procedure and/or visit. Please contact the physician who requested the consultation or procedure for results or contact Shore Heart Group directly.
Our office will notify patients of test results ordered by our physicians.
Patients who call with a non-emergency medical inquiries will be connected to one of our nursing staff, he/she will record your question in your chart, review the question with the physician and call you back as soon as possible with the doctor’s response.
If a family member has questions concerning a hospitalized patient, they should contact the patient’s doctor at the closest Shore Heart Group office. Large families are urged to appoint a family spokesperson to talk with the attending Shore Heart Group physician. |

What are the Shore Heart Group billing policies and procedures? |

All changes are due and payable in full upon receipt of a statement. Patients who have medical insurance should remember that professional services are rendered and charged to the patient, not to the insurance company. As a courtesy to our patients, we will submit insurance claims to the primary carrier. It is the patient’s obligation to know the terms of the insurance coverage to provide the practice with all information that is necessary to process a claim. For patients not covered by insurance, individual arrangements can be made for monthly payment plans. HMO patients who need referrals from their primary care physicians are responsible for obtaining these referrals. Patients will not be seen, nor will their diagnostic testing be performed without the appropriate referral. |

Shore Heart Group has 5 offices located in Monmouth and Ocean County; offices are located in Neptune, Brick, Toms River as well as Manahawkin and Freehold. Each office is a state-of-the-art facility which offers a full host of non-invasive testing, as well as consultative cardiology. 2D echocardiography, holter monitoring, arterial and venous ultrasound is available in all 5 offices. The offices in Toms River and Brick have state-of-the-art nuclear stress imaging capability. |

Which of the local area hospitals should I go to in the case of an emergency? |
| A. |
Shore Heart Group physicians are on staff at Jersey Shore Medical Center in Neptune, Ocean Medical Center in Brick, Community Medical Center in Toms River, as well as Southern Ocean County Hospital in Manahawkin. In the event of an emergency, you are advised to call 911 and present yourself to the closest emergency room. Please ask for a Shore Heart Group physician and you will be attended to by one of the physicians of the group on an emergent basis.
If you have non-emergent symptoms, please call the office on a 24-7 basis, wherein, you will promptly speak to the staff of Shore Heart Group and if necessary, the physician who is in the office at that point in time. If you require a phone call from your personal physician of Shore Heart Group, then he will be paged to answer your question as related to your care. |

Shore Heart Group has state-of-the-art nuclear stress test capability in Brick, Toms River and Neptune offices. Again, patients are encouraged to go to the office for complete cardiovascular evaluation which is nearest to their home. |

It is important to the staff of Shore Heart Group, as well as the physician members that the patients are satisfied with the quality of care that they receive, as well as the harmonious care which is therein provided by non-medical personnel. Therefore, if there is any unhappiness by the patient, this should be brought to the attention of the Manager who is located in each individual Shore Heart Group office. |

What are the office hours of Shore Heart Group? |

Shore Heart Group offices are open from 8:30 a.m. to 5:00 p.m., Monday through Friday. There will be diagnostic testing which will be performed on a Saturday basis; the times of this diagnostic testing will be forthcoming. |
